3 steps to changing your home care provider

The process of moving your Home Care Package to Home Care Assistance Gold Coast involves three simple steps, and we can help you at every stage. We can even schedule a visit to your home with one of our Care Managers who can complete all three steps right then and there with you.

Step 1

You will need to inform your current provider in writing of your wish to change providers. You will need to give them 2-4 weeks’ notice, depending on your agreement with them. If you are unsure of the notice period in your agreement, we can help you work this out and can also assist you to write the letter.

Step 2

Once you have notified your provider of your intent to change, you need to call My Aged Care on 1800 200 422 and let them know. They can then reactivate your Home Care Package referral code which will be used by your new provider. Please ensure you take note of this code.

Step 3

Lastly, simply call our friendly team at Home Care Assistance Gold Coast, provide us with your referral code and the date you will discontinue care with your current provider. One of our Client Care Managers will come out to see you to set up your new services, ensuring a seamless transition from your previous provider.
